About The Position

The Hospice RN Case Manager at Anchor Health is responsible for planning, organizing, and directing hospice care services tailored to the needs of patients and their families. This role involves assessing patient conditions, developing care plans, and collaborating with interdisciplinary teams and community physicians to ensure comprehensive patient care. The position emphasizes a supportive and compassionate work environment, aiming to make a genuine difference in the lives of patients and their families.

Requirements

• Graduate of a National League of Nursing accredited school of nursing.
• Current licensure in the State of California and CPR certification.
• Minimum of one year of experience as a professional nurse within the last three years, or a baccalaureate degree in nursing and a current RN license.
• Preferred: Minimum of two years' experience, with at least one year in public health, home care, or hospice nursing.
• Excellent observation, verbal and written communication skills, problem-solving skills, and basic math skills.
• Ability to lift, position, and transfer patients, and lift supplies and equipment.

Nice To Haves

• Management experience (not required but preferred).

Responsibilities

• Provide professional nursing care to patients utilizing all elements of the nursing process.
• Assess and evaluate patient's status and initiate the plan of care, making necessary revisions as needed.
• Develop a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions, involving the patient and family in the planning process.
• Act as Case Manager when assigned, coordinating patient care for an assigned caseload.
• Complete, maintain, and submit accurate clinical notes regarding patient's condition and care given.
• Communicate with physicians regarding patient needs and report changes in condition, obtaining necessary orders.
• Coordinate care plans with community health-related personnel.
