About the position

The RN Case Manager in Hospice at Access Hospice Care is responsible for delivering comprehensive nursing care to hospice patients. This PRN position emphasizes the importance of building trusting relationships with patients and their families while providing end-of-life care. The role involves evaluating patient needs, coordinating care plans, and ensuring high-quality service delivery in a supportive and empathetic environment.

Responsibilities

• Assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient.
• Conduct initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
• Identify and reassess the patient's/family's physical, psychosocial, and environmental needs at least every 15 days.
• Initiate and coordinate the plan of care while maintaining accurate clinical documentation.
• Collaborate with patients, families, attending physicians, and other team members in daily care provision.

Requirements

• Current RN licensure in the state of practice.
• One year of clinical experience in nursing.
• Current CPR Certification.
• Current driver's license and valid vehicle insurance, or access to public transportation.
