About the position

The Hospice RN (On-Call Nights) position at Tabitha is a part-time role focused on providing immediate evaluation and direct care to seniors in hospice settings. The RN will ensure continuity of care on a 24-hour basis, responding to after-hours referrals and delivering essential nursing services to enhance the quality of life for clients and their families.

Responsibilities

  • Responsible for immediate evaluation and direct care of seniors/residents/clients.
  • Provides emergency service to hospice seniors/clients.
  • Assesses and evaluates senior/client status by writing and initiating a plan of care.
  • Regularly re-evaluates senior/client and family/caregiver needs and participates in revising the plan of care as necessary.
  • Develops a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions.
  • Includes the senior/client and family in the planning process.
  • Uses health assessment data to determine nursing diagnosis.
  • Assists the senior/client with activities of daily living and facilitates efforts toward self-sufficiency and comfort care.
  • Acts as Case Manager when assigned by Patient Care Coordinator for assigned caseload.
  • Completes, maintains, and submits accurate clinical notes regarding senior/client condition and care given.
  • Records pain/symptom management changes/outcomes as appropriate.
  • Communicates with community health-related persons regarding senior/client needs and reports changes in condition; obtains/receives physicians' orders as required.
  • Teaches senior/client and family/caregiver self-care techniques as appropriate.
  • Provides medication, diet, and other instructions as ordered by the physician and utilizes opportunities for health counseling with seniors/clients and families/caregivers.
  • Works in concert with the interdisciplinary team.
  • Willingly takes on additional duties and/or assignments as necessary.

Requirements

  • Must be able to speak, read, write, and understand the English language including medical terminology related to the job.
  • Ability to prepare legible, accurate, and thorough documentation as required.
  • Ability to read a map and/or utilize a GPS system to find senior/client homes.
  • Successful completion of a Registered Nurse course of study; Bachelor's degree in Nursing preferred.
  • Two years of acute care/long-term care nursing experience within the last five years required.
  • RN License in good standing as recorded on the Nebraska State Registry or other Nurse Compact state required.
  • CPR certification required.
  • Valid Nebraska Driver's license and a good driving record required.
  • Must possess auto insurance at or above Nebraska statutory requirements.

Nice-to-haves

  • Experience in community health and/or hospice preferred.
  • Emergency Room/Critical Care experience is preferred.
