About the position
The Hospice RN Case Manager is responsible for admitting patients to hospice services, assessing their needs, and developing individualized care plans. This role involves coordinating care with a multidisciplinary team and participating in quality assessment programs, ensuring comprehensive support for patients and their families.
Responsibilities
- Admit patients to hospice services.
- Assess the physical, psychosocial, environmental, safety, and developmental needs of patients and families.
- Develop individualized plans of care based on assessments.
- Document observations and interventions related to the patient's environment.
- Coordinate care with a multidisciplinary team.
- Participate in the hospice program's quality assessment performance improvement program.
- Conduct follow-up visits as needed.
Requirements
- Valid RN License required.
- One year of hospice experience required.
- BLS Certification preferred.
