About the position

As an RN Case Manager at Moments Hospice, you will play a crucial role in providing compassionate care to patients in their final days, weeks, and months. This position offers autonomy and variety, allowing you to develop and implement personalized care plans while coordinating with interdisciplinary teams to ensure the comfort and well-being of patients and their families.

Responsibilities

  • Perform initial client assessments and develop and initiate the plan of care.
  • Make necessary revisions to the plan of care as the patient's status and needs change.
  • Develop a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions.
  • Include the patient and family in the planning process.
  • Counsel the patient and family in meeting nursing and related needs.
  • Coordinate patient care for assigned caseload.
  • Complete and submit accurate and relevant clinical notes regarding the patient's condition and caregiving.
  • Attend interdisciplinary meetings and coordinate interdisciplinary services.
  • Obtain physician orders for appropriate care.
  • Administer nursing cares and education.
  • Communicate continuously with patients, families, and other care team members.

Requirements

  • Licensed as a Registered Nurse / RN in the state you work in.
  • Proficiency with Electronic Medical Records (EMR) software is necessary.
  • Prior nursing experience in a TCU, hospice, or home health setting is preferred.
  • Valid Driver's License and reliable vehicle with current insurance.
  • Excellent at planning, organizing, and communicating.
  • Demonstrated ability to work autonomously.
