The Hospice Scheduler plays a crucial role in coordinating patient services and maintaining accurate patient records. Responsibilities
- Maintains a current patient roster with necessary information.
- Assists in coordinating services provided to patients.
- Enters patient data into electronic health record system.
- Performs workflow as assigned to role within EMR.
- Communicates with referral sources to confirm key patient information including missing data.
- Brings patient issues to the appropriate team member's attention.
- Identifies the need of callers and transfers them to the appropriate person.
- Performs other duties as needed.
Requirements
- Familiarity with medical terminology and procedures is a plus.
- Proficient in using office software, such as Microsoft Office Suite.
- Excellent communication and interpersonal skills.
- Strong attention to detail and organizational abilities.
- Ability to multitask and prioritize tasks effectively.
