About the position
As a Hospice Social Worker at Elara Caring, you will provide essential support and care to patients and families during their hospice journey. This role emphasizes personalized care, emotional support, and resourceful solutions, allowing you to make a significant impact in the lives of those you serve. You will work collaboratively with a dedicated team to ensure quality care and support for patients and their families.
Responsibilities
- Offer expert counseling and compassionate support to patients and their families.
- Guide individuals through grief, loss, and emotional challenges with professionalism and empathy.
- Conduct assessments to understand patients' psychosocial, financial, and environmental needs.
- Collaborate with interdisciplinary teams to develop and maintain individualized care plans.
- Communicate significant patient updates with care teams, ensuring quality and continuity of care.
- Deliver skilled social work interventions that align with professional standards and patient goals.
- Educate families about available resources, safety planning, and coping strategies.
Requirements
- A Master's or Doctoral degree in Social Work from an accredited institution.
- Active state licensure in Social Work.
- At least one year of experience in a healthcare setting.
- Strong interpersonal and social work skills aligned with industry standards.
- Valid driver's license, insurance, and reliable transportation with the ability to travel up to 50%.
