About the position

The Hospice Team Coordinator at Three Oaks Hospice is responsible for facilitating communication functions within the business office and providing essential clerical support. This full-time position requires effective customer service skills and the ability to manage various administrative tasks to support the hospice care team and ensure smooth operations.

Responsibilities

  • Facilitates the communications system including mail distribution, phone system, and memo handling.
  • Answers and triages incoming calls, documenting and disseminating appropriate messages.
  • Provides clerical support including preparation of admission, orientation, and new hire packets.
  • Monitors and orders office supplies and forms, and performs data entry and filing tasks.
  • Creates morning/end of day reports and agendas for meetings, and faxes weekly plans of care to external physicians.
  • Orders and organizes office and medical supplies as needed.
  • Promotes the agency philosophy and mission by presenting a positive image to patients, families, and community agencies.
  • Uses effective customer service skills when directing calls and assisting others.
  • Prepares monthly calendars, certifications, schedules, and contracts as needed.
  • Assists the team with admitting, discharging, and changing levels of care in the system.
  • Sets up continuous care, transportation, and orders medical equipment as required.
  • Assists with Medicaid/Medicare forms and reconciliations.

Requirements

  • High school diploma or equivalent; Bachelor's degree preferred.
  • Minimum of 1-year medical office or medical business experience; hospice office experience preferred.
  • Proficiency in MS Office, with advanced skills in Excel and PowerPoint preferred.
  • Excellent written and verbal communication skills.
  • Ability to work with a high sense of urgency.

Nice-to-haves

  • Experience in hospice care (2 years preferred).
