About the position

The Hospice Team Manager leads an interdisciplinary team providing end-of-life home hospice care to patients and families in New York. This role involves managing, coordinating, and supporting team members to ensure high-quality hospice services while complying with regulatory requirements. The position emphasizes clinical and quality improvement initiatives and requires effective management of day-to-day operations within the hospice care program.

Responsibilities

  • Lead and manage an interdisciplinary team providing hospice care to patients and families.
  • Ensure the implementation of clinical and quality improvement initiatives in compliance with regulatory requirements.
  • Participate in team meetings, family meetings, and patient calls to assess needs and provide support.
  • Monitor and audit files, processes, and procedures to ensure regulatory compliance.
  • Collaborate with Quality Improvement and Education to maintain professional practice standards.
  • Assist in the analysis of clinical, regulatory, and fiscal practices to support hospice care goals.
  • Manage day-to-day operations, including caseload management and achievement of productivity targets.

Requirements

  • License and current registration to practice as a Registered Professional Nurse in New York State.
  • Bachelor's degree in nursing or a related field, or equivalent work experience.
  • Minimum three years of clinical nursing experience, preferably in a home care or hospice setting.
