About the position
The Hospice Weekend RN position at Phoenix Home Care and Hospice involves providing skilled nursing care to clients in their homes over the weekends. The role requires strong assessment and communication skills to develop and implement individualized care plans in collaboration with a multidisciplinary team. The RN will work closely with other healthcare professionals to ensure the highest quality of care for hospice patients.
Responsibilities
- Assists in development, review, and revision of the plan of care.
- Provides nursing care in accordance with physician's plan of treatment and individual plan of care.
- Observes, records, and reports reactions to treatment and any changes in client's condition.
- Provides ongoing instruction and supervision of client, family, or other health team personnel in the plan of treatment.
- Participates in education programs, quality assessment activities, nurse's meetings, and other staff meetings as required.
Requirements
- Kansas or Compact Nursing License.
- Hospice experience required.
- Driver's License and Vehicle with Current Auto Insurance.
- Strong Organizational and self-management skills.
- Excellent Communication Skills.
