About the position

The Hospital Care Coordinator - RN Case Management at BayCare is responsible for providing comprehensive care coordination for patients, ensuring they achieve and maintain optimal functioning. This role involves linking patients to resources, managing caseloads, and coordinating discharge planning efforts in a fast-paced healthcare environment.

Responsibilities

• Provides linkage, monitoring, planning, and advocacy for patients to achieve and maintain maximum functioning.
• Effectively maintains and manages an assigned caseload based on the needs, strengths, abilities, barriers, and preferences of individuals served.
• Coordinates discharge planning efforts through assessment, referral, and interdisciplinary planning.
• Adjusts to changing priorities in a fast-paced environment.

Requirements

• RN License
• Bachelor's degree in Nursing (preferred)
• 2 years of experience in Acute Care
• 2 years of experience as a Registered Nurse
• Accredited Case Manager certification (preferred)
• Certified Case Manager certification (preferred)
• Associate Degree in Nursing (required)
