About the position
The Hospital at Home Hospitalist at South Shore Hospital plays a crucial role in delivering high-quality care to patients transitioning from the hospital to their homes. This position involves working within a multidisciplinary team to provide innovative care in a flexible environment, with opportunities for both in-person and remote work.
Responsibilities
- Admitting patients to the Hospital at Home unit.
- Providing direct patient care.
- Working collaboratively with a multidisciplinary team.
- Participating in Hospital at Home patient acquisition through partnership with the HAH Intake RNs.
- Developing appropriate plans of care and follow-up based on diagnostic assessments and physical examination findings.
- Ordering medications and writing prescriptions according to policies and procedures.
- Participating in daily HAH Interdisciplinary Rounds (IDR).
- Providing oversight of HAH Advanced Practice Clinicians.
- Performing virtual nighttime coverage as needed, on a rotating schedule.
- Participating in clinical onboarding and training of HAH staff.
Requirements
- Medical Degree from an accredited institution (MD/DO).
- Internship and Residency in Internal Medicine or Family Medicine from an accredited institution.
- BC/BE in Internal Medicine or Family Medicine.
- Unrestricted Physician License from the Board of Registration in Medicine.
- Massachusetts Controlled Substance Registration and DEA Registration.
- Active ACLS certification.
- Fluency in English, both verbal and written.
- Ability to learn how to use Epic EMR.
