About the position

The Housing and Community Support Specialist at Boston Medical Center's Living Well at Home Program is responsible for providing high-quality housing case management services to clients facing housing issues or experiencing homelessness. This role involves engaging with complex patients, advocating for their needs, and helping them stabilize in independent housing while collaborating with an interdisciplinary team.

Responsibilities

  • Provide case management services to victims and those at risk of community violence.
  • Engage and enroll complex patients into services.
  • Advocate for patients and provide specialty services to support housing readiness.
  • Support patients in identifying and obtaining housing opportunities.
  • Develop interdisciplinary care plans based on patient needs.
  • Facilitate access to social service resources and monitor patient progress.
  • Problem-solve with patients to enhance access to housing and community-based supports.
  • Provide community-based one-on-one support in collaboration with family and healthcare teams.

Requirements

  • Bachelor's degree in a behavioral health or related field, or two years of relevant work experience, or lived experience of homelessness or behavioral health conditions.
  • Minimum of 2 years prior healthcare, public health, or social services work in a community-based setting.
  • Prior experience working with individuals experiencing homelessness, mental illness, substance use disorder, or chronic health conditions.

Nice-to-haves

  • Fluency in Haitian Creole or Spanish.
  • Knowledge of Boston area resources and housing systems.
  • Understanding of social determinants of health affecting the patient population.
