About the position

The Housing Case Manager at Community Housing Innovations Inc. is responsible for developing and implementing housing and service plans for residents to secure and maintain permanent housing. This role involves providing direct assistance in housing searches, managing resident behaviors, and promoting a supportive environment. The position requires collaboration with various community services to ensure residents achieve independence and stability in their housing situations.

Responsibilities

  • Develop Independent Living Plans (ILPs) with customized housing goals for residents.
  • Assist residents in searching and applying for housing, including completing applications and rental subsidy programs.
  • Prepare residents for housing interviews and lease signings, and escort them to these appointments.
  • Connect residents to health, mental health, childcare, and financial assistance services.
  • Coordinate with immigration services and advocacy organizations for residents needing support.
  • Provide counseling, crisis intervention, and case management to residents.
  • Conduct weekly assessments of residents' progress with their housing and service plans.
  • Document interactions and communications with residents in the CARES system.

Requirements

  • Bachelor's degree in social services (social work, human services, sociology, psychology, or human relations).
  • Two years of experience in case management, housing services, or direct advocacy in social service settings.
  • Experience working with immigrants and immigration issues is prioritized.
  • Excellent verbal and written communication skills, with proficiency in English required and Spanish preferred.
  • Basic competency in MS Word, Excel, Outlook, and internet usage.
  • Ability to formulate simple financial budgets.
  • Experience working with homeless individuals and those with health or mental health issues.

Nice-to-haves

  • Master's in Social Work (MSW) preferred.
  • Experience with housing plans and service plan development.
  • Ability to engage residents effectively.
