About the position

The Housing Navigator/Case Manager at Titanium Healthcare is responsible for providing support and assistance to individuals and families experiencing homelessness or at risk of losing their housing. This role involves developing individualized housing plans, maintaining relationships with community resources, and assisting members with their housing search and tenant rights. The position is hybrid, allowing for both remote work and field service.

Responsibilities

  • Research, establish, and maintain a list of available housing opportunities in the region.
  • Develop relationships with local community resources including transitional housing, shelters, and permanent housing.
  • Develop individualized housing plans and communicate them to members.
  • Assist members with their housing search, including applying for apartments and negotiating with landlords.
  • Help members understand tenant rights and responsibilities.
  • Act as a mediator between members and landlords in case of disagreements.
  • Conduct follow-up meetings with members after housing placement to assist with issues.
  • Maintain accurate documentation of service objectives and outcomes in data systems.
  • Participate in collaborative team meetings to provide input and develop best practices.
  • Utilize the HMIS to add members into the housing portal and assist with CES assessments.

Requirements

  • Minimum 1-2 years of experience in healthcare, social work, public health, or related field, OR a bachelor's degree in healthcare.
  • Experience in social work, case management, or working with unhoused individuals and families.
  • Knowledge of case management systems and planning techniques.
  • Current and valid Driver's License with proof of auto insurance.

Nice-to-haves

  • Bilingual in Spanish.
  • Prior experience working with HMIS preferred.
