As a Case Manager, you will play a crucial role in delivering personalized support and advocacy for clients navigating various housing challenges. This position requires a compassionate individual with strong organizational skills and a commitment to social justice and community empowerment. In this role, you will provide outreach and engagement services to assigned clients, initially engaging them and maintaining working relationships. You will complete appropriate assessments and develop goal planners with client participation, facilitating linkages to collaborative resources when appropriate. You will assume primary responsibility for coordinating client goal plans, which may include coordinating mental health treatment, substance abuse treatment, and facilitating access to food, clothing, housing, medical care, financial assistance, and vocational training or employment. Advocacy is a key component of this position, as you will ensure that clients have access to community resources and services, meeting their needs and maintaining their rights. You will consult and collaborate with community providers to ensure continuity of care, and provide or arrange transportation to necessary appointments that align with client goals. Local travel is required to outreach clients, which may involve meeting them in the community, making home visits, and assisting with home inspections. Crisis intervention and risk assessment may also be necessary, as well as the ability to tolerate unusual or unpleasant behaviors. You will document services provided within required timeframes as outlined by funding contracts, program, and agency expectations. Coordination with internal leadership and team members is essential to ensure clear and accurate communication regarding case information and schedules. Additionally, you will plan, structure, and support community-based activities such as obtaining and setting up housing, grocery shopping, nutrition assistance, house cleaning, and other household activities. You will also assist with money management, social activities, and work-related support such as job coaching. Coordination with outside service providers, including inpatient and outpatient treatment services, state vocational rehabilitation services, social security, veterans' benefits, legal and advocacy services, and personal primary care medical and dental providers, will be part of your responsibilities. Your role will involve providing practical help and support, advocacy, coordination, side-by-side individualized support, problem-solving, direct assistance, training, and supervision to help participants obtain the necessities of daily living, including medical and dental health care, legal and advocacy services, financial support such as entitlements or housing subsidies, money management services, and transportation.

Responsibilities

Requirements
