About the position

Express Employment Professionals is seeking a dedicated Housing Services Case Manager to join a nonprofit organization focused on providing housing solutions and support services to individuals and families in need. This role is essential in delivering personalized support and advocacy for clients facing various housing challenges. The Case Manager will work closely with clients to empower them in creating sustainable pathways to stable housing and thriving communities. As a Case Manager, you will be responsible for outreach and engagement services to assigned clients, ensuring that initial contact is made and maintaining ongoing working relationships. You will complete appropriate assessments and develop goal planners in collaboration with clients, facilitating linkages to collaborative resources as needed. The role involves coordinating client goal plans, which may include mental health treatment, substance abuse treatment, and access to essential services such as food, clothing, housing, medical care, financial assistance, and vocational training or employment. Advocacy is a key component of this position, as you will work to ensure clients have access to community resources and services, maintaining their rights and needs. You will also provide or arrange transportation to necessary appointments that align with client goals. The role requires local travel to outreach clients, including meeting them in the community, making home visits, and assisting with home inspections. Crisis intervention and risk assessment may also be necessary, along with documenting services provided within required timeframes as outlined by funding contracts and agency expectations. Collaboration with internal leadership and team members is crucial to ensure clear communication regarding case information and schedules. You will also plan and support community-based activities, helping clients with housing setup, grocery shopping, nutrition assistance, and other daily living necessities. Coordination with outside service providers, including treatment services, vocational rehabilitation, and legal advocacy, is also part of the role. Overall, the Case Manager will provide practical help, advocacy, coordination, and individualized support to assist participants in obtaining the necessities of daily living.

Responsibilities

  • Provide outreach and engagement services to assigned clients, to initially engage clients and maintain working relationships.
  • Complete appropriate assessments, develop goal planners with client participation; facilitate linkages to collaborative resources when appropriate.
  • Assume primary responsibility for coordination of client goal plans, including coordinating mental health treatment and substance abuse treatment.
  • Advocate for clients' access to community resources and services, ensuring that clients' needs are met and rights maintained.
  • Provide or arrange transportation to necessary appointments that meet client goals.
  • Travel locally to outreach clients; meeting them on the street/in the community, make home visits, and assist with home inspections.
  • Provide crisis intervention and risk assessment as necessary, and tolerate unusual or unpleasant behaviors.
  • Document services provided within required timeframes as outlined by funding contract, program and/or agency expectations.
  • Coordinate with internal leadership and team to ensure clear and accurate communication relevant to case information and schedules.
  • Plan, structure and support community-based activities such as obtaining and setting up housing, grocery shopping, nutrition assistance, and other household activities.
  • Coordinate with outside service providers including inpatient and outpatient treatment services, state vocational rehabilitation services, social security, veterans' benefits, legal and advocacy services, and personal primary care medical and dental providers.
  • Provide practical help and support, advocacy, coordination, side-by-side individualized support, problem-solving, direct assistance, training, and supervision to help participants obtain the necessities of daily living.

Requirements

  • Ability to establish and maintain effective working relationships with staff, community partners, and professionals.
  • Strong time management and interpersonal skills.
  • Reliable transportation, WA State Driver's License & proof of current automobile insurance.
