About the position

The Housing Stability Case Manager is accountable directly to the Director, Housing Programs, or the Team Coordinator within their respective program as assigned. This is a full-time position based out of Martin Luther King Jr. Ave. SE. The salary range for this position is $25.96 – 26.92 per hour and the offer amount is determined by the candidate's education, qualifications, and experience.

Responsibilities

  • Support families in gaining and maintaining housing stability through developing case management plans that incorporate goals for both adults and children related to core areas: finding stable housing; increasing income; and family stabilization and support.
  • Meet face to face with families on caseload in their homes to pursue goal-related tasks, review progress, and address barriers to accessing services.
  • In collaboration with families and Housing Specialist, assist with housing search and placement process, sometimes accompanying families to view units, advocating for families with landlords, and assisting families to successfully complete the application process.
  • Network with community resources for housing, employment, emotional wellness services, training, etc., and connect families as appropriate, including to COH health services.
  • Engage families and assess needs, preferences, and strengths.
  • Demonstrate empathy and flexibility in responding to families’ needs and requests while fostering independence.
  • Act as a liaison with landlords and other community supports as needed.
  • Participate in multidisciplinary meetings, trainings, and regular supervision.

Requirements

  • Bachelor’s Degree with a minimum of 1 year of experience in case management preferred.
  • In lieu of the education requirement, a minimum of two years of experience in a social service and/or human service field.
  • Strong knowledge of community resources or ability to become knowledgeable required.
  • Strong interpersonal and organization skills required.
  • Strong communication skills including verbal and writing.
  • Proven ability to submit timely, detailed and accurate case notes.
  • A valid driver’s license and regular access to a car, as well as proof of auto insurance, required for staff working in scattered site housing programs.
