About the position

The Human Services Coordinator I (HSC) position at the Massachusetts Department of Mental Health (DMH) Metro Boston Fuller Site is designed for individuals with strong attributes in the Human Services field. The role focuses on assisting adults eligible for DMH services to maintain stability in the community by providing resources, crisis stabilization, and mental health assessments. The HSC will work closely with clients to ensure they receive the necessary support and services to thrive in their environment.

Responsibilities

  • Carries a case management caseload to help clients locate and maintain housing, financial assistance, medical, mental health and dental services, vocational and educational resources, socialization opportunities, and transportation.
  • Develops Individual Service Plans (ISP) for clients to ensure coordination and monitoring of all aspects of treatment.
  • Provides outreach and crisis intervention to assess mental health needs and ensure proper psychiatric treatment.
  • Maintains extensive liaison work with community agencies to maximize the quality of life for clients.
  • Collects standardized client-related data for research, evaluation, or administrative purposes.
  • Maintains up-to-date client records, including progress notes and ISP, to ensure adequate treatment services.
  • Visits clients in their homes or usual places of residence to monitor safety and well-being.
  • Facilitates referrals to programs best suited to clients' service needs and coordinates transitions to various community programs.
  • Safeguards human rights of clients through support, advocacy, and reporting of human rights violations.
  • Performs related duties as assigned.

Requirements

  • Knowledge of psychosocial problems of those with mental/emotional disorders.
  • Awareness of principles, practices, and techniques of counseling.
  • Knowledge of programs and services available to the mentally ill population.
  • Knowledge of principles and practices of crisis intervention.
  • Capacity to evaluate and maintain accurate records.
  • Ability to make decisions quickly and maintain a calm manner in stressful situations.
  • Ability to write concisely and express thoughts clearly.

Nice-to-haves

  • Experience working with individuals with serious, persistent mental illness, substance abuse, and/or who have experienced homelessness.
  • Bilingual or multilingual fluency.
