About the position

The Licensed Behavioral Health Provider plays a crucial role in delivering psychosocial support to individuals and families dealing with chronic, acute, or terminal illnesses. This position involves providing patient education, counseling, and referrals to other services, while also offering care and case management to promote health and address barriers to healthcare access. The provider works collaboratively within a patient-centered medical home care team to ensure optimal patient outcomes.

Responsibilities

  • Evaluate and assess patients to determine their needs and support options.
  • Refer patients or families to community resources for recovery assistance.
  • Provide individual sessions to assist patients with behavioral health issues.
  • Collaborate with other care providers to coordinate integrated services.
  • Utilize consultation data to plan and coordinate patient care and rehabilitation.
  • Modify treatment plans based on changes in clients' status.
  • Monitor, evaluate, and record patient progress according to treatment goals.

Requirements

  • Master's level degree in social work or related field.
  • Valid license in social work or counseling (LCSW, LMFT, LPCC, ASW).
  • One to two years of experience in clinical social work or counseling.
  • Knowledge of human behavior, psychological research methods, and treatment of behavioral disorders.

Nice-to-haves

  • Bilingual fluency in English and Spanish.
