About the position

We are seeking a dedicated and detail-oriented Registered Nurse (RN) to join our team as a UAS Assessor. This full-time position offers flexibility and autonomy, allowing RNs to achieve a work-life balance while conducting health assessments to determine patient eligibility for home care services. The role involves both in-person and telehealth assessments, with comprehensive training provided to certify RNs as UAS Nurse Assessors.

Responsibilities

• Conduct health assessments to determine patient eligibility for home care services.
• Complete three assessments per day as a daily quota, with the potential for bonuses for additional assessments.
• Travel within the assigned borough to conduct in-person assessments or utilize telehealth modalities.
• Document assessment findings accurately and efficiently.
• Maintain communication with patients and healthcare teams.

Requirements

• Valid RN License.
• BLS Certification.
• Excellent written documentation skills.
• Strong computer skills.
• Access to a personal vehicle or public transportation.

Nice-to-haves

• Bilingual in Spanish and English is preferred.
• Experience in home health or long-term care settings.
