About the position

The IDD Service Coordinator position at Bluebonnet Trails Community Services is designed to provide essential services to children and adults with developmental disabilities. The role focuses on assessing individual needs, developing service plans, facilitating service linkages, and monitoring the effectiveness of these services. The coordinator will advocate for individuals, assist in crisis intervention, and work collaboratively with families and various agencies to empower individuals to achieve their life goals and integrate effectively into their communities. This position requires a proactive approach, independent judgment, and the ability to work with minimal supervision while maintaining sensitivity to cultural differences.

Responsibilities

  • Provide services to children and adults with developmental disabilities based on the Person-Centered Thinking model.
  • Conduct comprehensive assessments of individual needs and desires.
  • Develop and implement service plans tailored to individual requirements.
  • Facilitate service linkages and monitor their effectiveness.
  • Provide crisis intervention and prevention supports.
  • Advocate for individuals in job readiness and employment activities through the Employment First project.
  • Participate in interagency planning and CRCG meetings to bridge gaps in services.
  • Document services provided and interactions with individuals and families.

Requirements

  • An associate degree or higher in social, behavioral, or human service field, or a high school diploma with two years of relevant experience.
  • Experience working with individuals with intellectual or developmental disabilities is preferred.
  • Ability to use good clinical judgment and work effectively in a fast-paced environment.
  • Demonstrated computer and documentation skills.
  • Knowledge of appropriate community resources.

Nice-to-haves

  • At least one year of experience working with individuals with intellectual/developmental disabilities.
  • Bachelors Degree in a relevant field.
  • Masters Degree in a relevant field.
  • Bilingual (English/Spanish/ASL) skills.
  • Experience with Medicaid programs.
