About the position

The IDD Service Coordinator at PermiaCare is responsible for monitoring and coordinating care and services for individuals with intellectual and developmental disabilities (IDD). This role involves assessing needs, developing service plans, facilitating service linkages, and providing crisis intervention and advocacy to ensure clients achieve their personal goals and maintain a high quality of life. The coordinator works independently under limited supervision, maintaining effective communication with clients, families, and service providers.

Responsibilities

  • Monitor care and services provided to individuals with IDD to ensure quality of life.
  • Conduct assessments to identify personal outcomes and service needs.
  • Develop and facilitate service plans and linkages to appropriate services.
  • Provide crisis intervention and prevention supports as needed.
  • Advocate for clients to ensure their needs and preferences are met.
  • Document progress and satisfaction with services in a timely manner.
  • Meet regularly with clients and their representatives to monitor outcomes.
  • Ensure compliance with local, state, and federal regulations regarding service delivery.
  • Maintain confidentiality of sensitive records and adhere to HIPAA regulations.
  • Participate in community activities and meetings as required.

Requirements

  • Bachelor's degree in psychology, social work, nursing, or related field.
  • At least 1 year of experience working with individuals with intellectual and developmental disabilities or mental illness preferred.
  • Valid Texas Driver's license and acceptable driving record.
  • Ability to pass criminal history and background checks.
  • QIDP certification must be obtained within 6 months of hire.
  • Strong organizational and time management skills.
  • Effective communication skills, both oral and written.

Nice-to-haves

  • Experience with case management or service coordination.
  • Knowledge of trauma-informed care principles.
  • Familiarity with Medicaid billing processes.
