About the position

The IDD Service Coordinator at Denton County MHMR Center plays a crucial role in supporting individuals with Intellectual and Developmental Disabilities (IDD) by providing comprehensive service coordination. Under the direction of the LIDDA Director, the coordinator operates within federal, state, and local guidelines to assess individual needs, develop and monitor Person Directed Plans (PDPs), and connect clients with necessary services and supports. This position requires a strong commitment to advocating for individuals with developmental disabilities, ensuring their rights are upheld, and promoting their choices in service delivery. The Service Coordinator will manage a varied caseload of clients with multiple and complex needs, conducting regular assessments and meetings to ensure that each individual's plan is tailored to their unique circumstances. Responsibilities include maintaining documentation of monthly client contacts, progress notes, and revisions to the PDP as needed. The coordinator will also facilitate initial and annual PDP meetings, ensuring that all stakeholders are informed and involved in the planning process. In addition to direct service coordination, the role involves collaboration with family members, service providers, and community agencies to foster positive relationships and ensure comprehensive support for clients. The coordinator must respond effectively to crisis situations, ensuring the health and safety of clients while adhering to the Center's policies and procedures. This position allows for partial remote work once training is completed and productivity measures are consistently met, providing flexibility in the work environment.

Responsibilities

  • Assess and determine individual needs for individuals with Intellectual and Developmental Disabilities (IDD).
  • Develop and monitor Person Directed Plans (PDP) for clients.
  • Provide ongoing services to assist individuals in achieving their desired outcomes.
  • Meet with clients at least monthly to monitor progress and provide assistance.
  • Document face-to-face monthly contacts and additional collateral contacts for Medicaid clients.
  • Complete progress notes within three business days of client meetings.
  • Hold initial and annual PDP meetings and revise plans as necessary throughout the year.
  • Advocate for individuals by reviewing rights and informing them of the complaint process.
  • Maintain positive relationships with clients, families, service providers, and community agencies.
  • Respond to individual crisis situations and ensure client safety.
  • Complete required documentation in accordance with Center policies and procedures.
  • Provide backup support to other service coordinators during absences or vacancies.
  • Independently schedule and make client contacts as needed.
  • Complete assigned HCS/TXHML enrollments within designated timeframes.

Requirements

  • Bachelor's degree from an accredited college or university in social, behavioral, or human service field.
  • At least one year of human services experience preferred.
  • Experience in LIDDA or Service Coordination preferred.
  • Valid Texas driver's license and auto liability insurance.
  • Ability to operate computers and applicable software effectively.
  • Strong communication skills, both orally and in writing.
  • Ability to organize and prioritize assignments while managing time effectively.
  • Desire to monitor and advocate for individuals in services.
  • Skilled in effective problem solving and conflict resolution.
