About the position

The Immediate Access to Care Coordinator at Family Guidance Centers, Inc. is responsible for conducting intake screenings and assessments for individuals seeking behavioral health services, particularly in substance use disorder treatment. This role involves coordinating the admission process, developing treatment plans, and providing direct clinical services to patients. The position emphasizes compassionate care and aims to facilitate positive, lasting change in the lives of individuals and families.

Responsibilities

  • Complete FGC intake screening documents via face-to-face or telehealth to determine appropriate level of care.
  • Administer the biopsychosocial assessments in EHR (SAMMS) with attention to clinical detail and justification from DSM.
  • Complete all intake paperwork including admission note within one business day of the intake.
  • Coordinate admission process with Retention Specialist to ensure successful completion of all steps by patient (i.e., toxicology, medical team/nursing).
  • Provide Naloxone and training to all intakes.
  • Develop patient treatment plans and conduct continued stay review based on research, clinical experience, and patients' histories using SAMMS assessment on computer.
  • Provide direct clinical service to a caseload to assist with CADC requirements.
  • Counsel patients individually, or in group sessions, to assist in overcoming dependencies, adjusting to life, or making changes.
  • Counsel family members to assist them in understanding, dealing with, and supporting patients (written authorization by patient must be obtained).
  • Plan or implement follow-up or aftercare programs for patients to be discharged from treatment programs.

Requirements

  • High School Diploma / GED required.
  • Experience in/with substance use, detox programs, medication-assisted treatment, behavioral healthcare and/or community relations encouraged to apply.
  • IAODAPCA Certified Alcohol and Other Drugs Counselor (CADC) required, or able to obtain within allotted time of hire date.
