About the position

The In-Home Medical Assistant (Field Care Coordinator) at One Medical plays a crucial role in providing care to patients outside of traditional clinic settings. This position involves visiting patients in their homes to facilitate telehealth visits, conducting clinical assessments, and supporting patients in navigating their care plans and community resources. The role emphasizes building trust and rapport with patients and their families, ensuring they receive comprehensive care and support during their healthcare journey.

Responsibilities

  • Facilitate telehealth visits in a patient's home using technology to ensure a complete view of the living situation.
  • Conduct standard patient assessments and documentation to facilitate a complete visit by the treating provider.
  • Perform clinical duties including telehealth setup services, health status screenings, and vital checks.
  • Coordinate care with patients, families, and outside care teams as needed.
  • Connect patients to community resources related to housing, transportation, and food.
  • Assist patients in navigating healthcare services and community resources.
  • Participate in daily operations of the High Risk Care Team, including answering calls and responding to emails.
  • Teach patients and family members to connect with their care teams via technology.

Requirements

  • At least 1 year of experience in home-based healthcare services or senior living care settings, or EMT experience.
  • Certified Medical Assistant (MA) and Basic Life Support (BLS) preferred or ability to obtain certification within 6 months of hire.
  • Strong written and verbal communication skills; Spanish speaking is a plus.
  • Proven ability to build rapport and trust with patients while delivering individualized care.
  • Clinical skills in performing basic vitals and clinical assessments preferred.
  • Proficiency in computer technology and video platforms.

Nice-to-haves

  • Experience facilitating access to community resources or social service agencies.
  • Experience with Mac iOS and Google Suite.
