About the position

The Independent Living Coordinator I at Kids Central, Inc. plays a crucial role in supporting young adults transitioning out of the foster care system. This position focuses on equipping these individuals with the necessary skills, knowledge, and resources to achieve successful independence. The coordinator serves as a primary case manager, ensuring that youth receive essential life skills training, educational support, and access to a network of supportive adults during their transition to adulthood.

Responsibilities

  • Serve as primary case manager for youth aged 18-23 and secondary case manager for youth aged 17.
  • Schedule Transition Meetings and My Pathways to Success Plans in the youth's home.
  • Complete quarterly home visits every 30 days with youth aged 17 in licensed foster care.
  • Ensure youth are receiving life skills training in their placement and track their progress on formal assessments.
  • Assist with educational workshops geared toward youth and young adults.
  • Participate in various youth functions after hours, including the Youth Advisory Council, Summer Camps, and IL Graduation Celebration.
  • Prepare and process necessary paperwork for youth participating in the Road to Independence Program, including Postsecondary Education Services and Support, Educational Training Voucher, Extended Foster Care, and Aftercare programs.
  • Prepare Case Plans and Judicial Reviews and attend court for young adults aged 18-22 in Extended Foster Care.
  • Attend court hearings for youth aged 17 in licensed care and prepare a Status Report for the court.
  • Monitor educational progress of youth and young adults to ensure success.
  • Monitor youth in the Extended Foster Care program in their placement.
  • Perform other duties as assigned.

Requirements

  • Master's degree preferred; Bachelor's degree in Human Services or related field required.
  • 2-3 years of Child Welfare experience preferred.
  • Child Welfare Certification preferred but not required at time of hire; must obtain within one year of employment.
  • Must maintain a clear criminal background and driving record.
  • Valid Florida Driver's license required with proof of appropriate automobile insurance limits.

Nice-to-haves

  • Experience in public speaking and mentoring youth.
  • Proficiency in Microsoft Office Suite (Excel, Word, Outlook, PowerPoint).
