About the position

As an Independent Optometrist at Sam's Club, you will lease space to provide eye and vision care services while maintaining full autonomy over your practice. This role allows you to set your own schedule, fees, and patient management, ensuring a personalized approach to patient care.

Responsibilities

• Provide comprehensive eye and vision care to patients.
• Establish your own days and hours of practice.
• Control your patient schedule and fee structure.
• Maintain 100% of exam and professional fees.
• Optionally utilize equipment and administrative support services.

Requirements

• Doctor of Optometry (OD) from an accredited optometry school with a valid state optometry license.
• Ophthalmologist (MD or DO) from an accredited school with a valid state ophthalmology license.
• General Liability Certificate.
• Professional Liability Insurance Certificate.
• W-9 form.
