About the position

As an Independent Optometrist leasing space inside Walmart stores, you will play a crucial role in providing eye and vision care while maintaining autonomy over your practice. This position allows you to establish your own schedule, control patient fees, and retain all professional fees from your services. You will have the option to utilize equipment and support services, depending on state regulations.

Responsibilities

• Provide comprehensive eye and vision care to patients.
• Establish your own practice days and hours of operation.
• Control your patient schedule and fee structure.
• Retain 100% of exam and professional fees.
• Optionally utilize equipment and administrative support services.

Requirements

• Doctor of Optometry (OD) from an accredited optometry school.
• Valid state optometry license.
• Ophthalmologist (MD or DO) from an accredited school (if applicable).
• Valid state ophthalmology license (if applicable).
• General Liability Certificate.
• Professional Liability Insurance Certificate.
• W-9 form.
