About the position

The Inpatient Case Manager RN at Sentara Norfolk General Hospital is responsible for providing comprehensive care management and facilitating quality outcomes for patients throughout their healthcare journey. This role focuses on ensuring appropriate patient transitions to various levels of care while promoting effective utilization of health services. The position requires collaboration with healthcare teams and patients to develop and implement care plans that address the physical, mental, and psychosocial needs of patients and their caregivers.

Responsibilities

• Provide and facilitate comprehensive care management services for patients across the healthcare continuum.
• Focus on patient transition to the appropriate level of care while ensuring clinical appropriateness and length of stay (LOS).
• Support in-hospital patients identified as needing comprehensive case management.
• Promote effective utilization and monitoring of health services.
• Collaborate and communicate with the healthcare team and patient/caregiver to manage care and transitions.
• Develop and implement comprehensive care plans based on assessment and evaluation.

Requirements

• 18 months nursing experience required.
• Registered Nurse (RN) license required.
• Basic Life Support (BLS) certification within 90 days of hire.
• Bachelor of Science in Nursing (BSN) preferred.
• Case Management certification (ACM, CCM, CCCTM, CMAC, or CMGT-BC) required within one year of eligibility.
• 1 year of acute care case management experience preferred.

Nice-to-haves
