About the position

Aiken Regional Medical Centers, located in Aiken, South Carolina, is a 273-bed acute care facility providing top quality and safe healthcare to the residents of Aiken and surrounding communities since 1917. Aiken Regional Medical Centers has been ranked a top hospital in South Carolina by the American Heart Association for its treatment of heart attack, heart failure and Stroke, and most recently, coronary artery disease. Additionally, Aiken Regional provides comprehensive healthcare services such as behavioral health (Aurora Pavilion Behavioral Health), emergency medical care (main hospital and ER at Sweetwater), orthopedic surgeries, maternity, rehabilitation services(Hitchcock Rehabilitation Services), imaging, and wound care. The Inpatient Rehab Case Manager (RN) is responsible for providing safe, competent nursing care for patients in accordance with facility policies, standards, and philosophy. This role includes ensuring effective communication of information to insurance companies, physicians, and post-acute care providers to ensure a smooth transition of care for all ARMC patients. The Case Manager will manage and coordinate case management and social work administrative activities as needed.

Responsibilities

  • Coordinates completion of and transmits information to community agencies to obtain a decision consistent with the transitional plan for care.
  • Contacts agencies regarding bed availability and accessibility, procedures for admission, schedules visits pertinent to the plan for the continuum.
  • Responsible for reconciliation of post-acute care discharge plans to ensure that Transfer DRG requirements are met.
  • Assists with completion of paperwork, as requested, by case managers and Director within the limits of role.
  • Assures seamless transitions for the patient/family across the continuum of care.
  • Provides daily, clear communication to the clinical staff regarding the status of the patients on their unit.
  • Fosters cordial, positive and professional interpersonal relationships with patients, family members, physicians, staff, nursing homes, community agencies, insurance companies and peers.
  • Acts as a proactive member of the interdisciplinary team.
  • Provides administrative support to the Director and other department staff.
  • Involves the patient and family psychosocial, spiritual, emotional, population specific, and diversity needs in care assessment.
  • Constructs and implements the plan of care using nursing knowledge and skill.
  • Coordinates the implementation of the interdisciplinary plan of care for assigned patients.
  • Consistently completes hourly rounds with purpose to strengthen nurse communication, improve safety, and anticipate patient needs.
  • Actively participates in bedside report to ease anxiety and address important points of care.
  • Provides patients with caregiver name and contact information each shift.
  • Provides care that is respectful of and responsive to the patient values in clinical decision making.
  • Supports and informs the patient and family of hospital specific components of the Patient Centered Care Delivery model.
  • Assumes role of team member, charge nurse or team leader based on the need and/or situation.
  • Follows communication practices that minimize risks associated with handoffs among providers.
  • Leads or participates in the design and implementation of systems that support effective teamwork.
  • Empowers contributions of others who play a role in helping patients/families achieve health goals.
  • Demonstrates the ability to formulate questions that address clinical issues.
  • Demonstrates proficiency in completing an evidence-based practice literature search.
  • Analyzes best evidence to address a clinical question.
  • Translates evidence into clinical practice through the consistent use of knowledge and evidence.
  • Demonstrates quality through accurate documentation of the nursing process.
  • Utilizes the results of quality improvement activities to initiate changes in nursing practice and care delivery.
  • Participates in efforts to minimize cost and eliminate waste and unnecessary duplication.
  • Demonstrates competent clinical skills, critical thinking, and nursing judgment.
  • Follows facility policies and procedures to promote patient safety.
  • Reports errors to promote analysis of errors and design of prevention or system improvements.
  • Utilizes appropriate patient safety strategies to prevent human errors.
  • Demonstrates effective use of technology and standardized practices to promote safety.
  • Navigates the patient's electronic health record to accurately enter and locate appropriate information.
  • Incorporates patient data from multiple sources to identify problems and assure timely and appropriate intervention.
  • Ensures the confidentiality and security of patient information while advocating for and involving the patient as appropriate.
  • Participates in the collection of data to support nursing research and evidence-based nursing practice.
  • Maximizes the capabilities of the electronic clinical information system to enhance clinical decision and care planning.
  • Makes discretionary patient care decisions/interventions independently within the scope of practice.
  • Practices interdependently, utilizing the skills and observations of team members appropriately.
  • Facilitates problem resolution among peers and unlicensed team members when behaviors and decisions are not within the range of expected actions.
  • Prioritizes and delegates care appropriately.
  • Evaluates changes in the patient condition and reports significant findings to the physician.

Requirements

  • Graduate of an accredited or NLN-approved RN program.
  • Current SC or qualifying compact state nursing license.
  • BLS required.
  • Rehab experience is preferred.
