About the position

The Intake Associate at Urban Health Plan is responsible for providing orientation and essential information to new patients, ensuring they understand how to access the full range of services offered. This role requires excellent communication skills and the ability to maintain patient confidentiality while conducting intake assessments and completing necessary documentation.

Responsibilities

  • Conduct intake for new patients at assigned site.
  • Complete patient assessment including screening questions.
  • Complete required new patient documentation.
  • Assure patient confidentiality.
  • Participate in quality improvement program.

Requirements

  • Bilingual in Spanish and English.
  • High school diploma or GED.
  • 1 year of experience in an office setting with clerical duties.
  • Excellent communication skills.
  • Understanding of phone etiquette and standard office procedures.
