About the position

The LPN at Avenir Behavioral Health Center is responsible for providing comprehensive patient care under the direction of the Charge Nurse. This role involves monitoring patient conditions, responding to requests, and ensuring compliance with treatment plans while maintaining a safe and therapeutic environment.

Responsibilities

  • Carry out all aspects of patient care as directed by the Charge Nurse.
  • Observe and communicate any specialized services needed to the Charge Nurse/RN.
  • Perform ongoing observation of patients to ensure active treatment and participation.
  • Respond appropriately and timely to patient requests.
  • Report changes in patients' conditions or behaviors to the Charge Nurse/RN.
  • Intervene in behavioral crisis situations using approved interventions.
  • Initiate and participate in emergency medical care when necessary.
  • Assist patients with daily living activities such as meals, grooming, and toileting.
  • Participate in the implementation of individualized care plans with the treatment team.
  • Provide patient education for identified learning needs.
  • Facilitate educational/activity groups as assigned.
  • Ensure compliance with infection control protocols and policies.
  • Complete and sign incident reports and provide follow-up as required.
  • Monitor patient environment for safety and sanitary regulations.
  • Review assignment sheets and communication tools each shift.
  • Review laboratory and test reports, practitioner orders, and medication administration records daily.
  • Facilitate a therapeutic and safe environment for patients.
  • Ensure all patient valuables are recorded and stored per policy during inpatient stays.
  • Document medication variances and adverse drug reactions as required.

Requirements

  • Ability to provide excellent service to internal and external customers.
  • Strong writing and communication skills.
  • Ability to observe and report patient conditions effectively.
  • Entry-level experience in patient observation and care.

Nice-to-haves
