About the position

The Intake Nurse Assessor (LPN) plays a crucial role in the Georgia Care Coordination Services by serving as the primary point of contact for individuals with complex care needs. This position involves conducting comprehensive needs assessments, developing personalized care plans, and ensuring continuity of care through collaboration with case managers and other healthcare professionals. The role is essential in providing options counseling and educating participants and their families about available care options and programs.

Responsibilities

  • Conduct outreach and schedule initial in-person visits for individuals with complex care needs.
  • Perform comprehensive needs assessments and complete follow-up visits as required.
  • Develop personalized care plans based on individual assessments.
  • Identify potential risks and safety concerns and report them as necessary.
  • Coordinate with ongoing Case Managers to ensure continuity of care.
  • Educate participants, caregivers, and family members on available care options and programs.
  • Collaborate with the Case Manager and leadership to address immediate concerns regarding potential abuse or neglect.

Requirements

  • Associate degree in nursing from an accredited institution.
  • Current and valid Licensed Practical Nurse (LPN) license.
  • Minimum of three (3) years of nursing experience, preferably in an intake, triage unit, or assessment role.
  • Experience in community health, home health, or case management is preferred.
  • Knowledge of Medicaid, Medicare, and/or waiver programs is preferred.

Nice-to-haves

  • Experience in a similar role or in community health, home health, or case management.
