About the position

The position involves collaborating with medical providers and other members of the care team to ensure coordination of care. The individual will serve as an independent mental health practitioner, responsible for diagnosing and treating mental health illnesses. The role includes providing brief interventions for patients in crisis and intensive care management for patients with acute or chronic complex needs. Additionally, the individual will work in collaboration with the clinical leader and physician leader to provide direction and support to the assigned pod, as well as educate residents and staff about behavioral health issues.

Responsibilities

  • Conducts bio-psycho-social mental health assessments of patients and develops patient-centered treatment plans.
  • Provides care management for patients with psychosocial and/or complex health needs with a goal of preventing Emergency Department use and hospitalization. Provides longer-term therapy as appropriate.
  • Monitors schedule in order to maximize patient access.
  • In collaboration with the nurse clinical leader and physician leader, serves as a resource for an assigned pod or patient population.
  • Participates in education and training of Behavioral Health Interns and Family Medicine Residents.
  • Documents care provided according to Medicare and other compliance standards, and CHMG and practice standards.

Requirements

  • Masters level education in Social Work, Marriage and Family Therapy, or Clinical Mental Health Counseling.
  • Licensed Independent Social Worker, Licensed Marriage and Family Therapist, Licensed Clinical Mental Health Counselor or Licensed Psychologist required; or required to obtain license within three years of hire date.
  • Two years experience in a medical setting is preferred.
