About the position

The Integrated Care Lead Therapist is responsible for coordinating team-based brief interventions and referrals in various healthcare settings, including outpatient services, inpatient services, telemedicine, and emergency rooms. This role addresses a wide range of social, behavioral health, and substance abuse treatment needs within a rural Alaska Native population. The therapist supervises integrated care and crisis clinicians to provide immediate and limited services, ensuring low-barrier access to behavioral health services.

Responsibilities

  • Supervises and coordinates the work of the integrated care team.
  • Provides clinical supervision and education where appropriate within scope.
  • Performs behavioral health, substance use, and social needs assessments in primary care and/or emergency room settings.
  • Performs screenings, brief interventions and referrals to treatment (SBIRTS).
  • Performs biopsychosocial assessments to handoff clients into community behavioral health care services.
  • Delivers interventions to address behavioral health and substance abuse needs.
  • Connects patients to social and tribal services to address relevant social needs.
  • Maintains awareness and promotes coordination of social, tribal, health, legal, community behavioral health, and substance abuse treatment services.
  • Communicates effectively with the care team, including providers, nursing staff, patient advocates, CHA/Ps, behavioral health aides, and case managers.
  • Participates in the crisis on-call rotation as a member of the behavioral health team.
  • Attends all meetings, trainings, and case staffing's, leads integrated care meetings.
  • Documents treatment process and progress in EHR to meet all relevant requirements and support well-coordinated patient care.
  • Maintains confidentiality of patient records and treatment.

Requirements

  • Must have a Master's degree in Social Work, Professional Counseling, Psychology, or Family and Marital Therapy (or the like).
  • Master's degree must be eligible for Alaska State licensure at the time of hire, and candidates must obtain their Alaska license within 6 months of hire.
  • Background working with rural American Indian/Alaska Native populations is preferred.
  • Medical social work experience is preferred.
  • Ability to work with sensitivity in a cross-cultural environment.
