About the position

The Intensive Care Manager, Overflow position at Mental Health Cooperative involves providing continuous treatment team care management services to children and youth with serious emotional disturbances or severe mental illness. The role includes mentoring new care managers and ensuring that consumers receive appropriate support and resources. This position is crucial in facilitating the transition of consumers from hospital settings and coordinating their ongoing care within the community.

Responsibilities

  • Continual assessment of consumers' needs and development of individualized service plans and crisis plans with measurable goals.
  • Assist with referral and linkage to community resources and advocate for consumers.
  • Participate in a rotating on-call schedule, including after hours and weekends.
  • Ensure consumers are seen upon discharge from the hospital and Diversion Services within Agency standards.
  • Plan and coordinate consumers' daily psychiatric, medical, and environmental needs.
  • Ensure efficient transition of consumers when needed within the Agency and the community.
  • Meet productivity standards as indicated in the Policies and Procedures.
  • Meet regularly with Care Management Supervisor (individual and group).
  • Attend morning and weekly team meetings as scheduled and follow up with the plans developed in each meeting.
  • Attend quarterly All Care Management meetings/trainings.
  • Facilitate education of consumers and/or involved collaterals regarding psychiatric diagnosis and appropriate treatment options.
  • Respond to urgent/crisis consumer needs with assistance from supervisor, crisis services, and clinic staff by developing plans and interventions.
  • Develop and maintain relationships within the family unit and supports.
  • Ensure timely and appropriate documentation as required by the Agency.
  • Develop and maintain community relations through prompt follow-through with contacts.
  • Continual involvement in the family system, school system, and other systems in which the family may be involved.

Requirements

  • Bachelor's Degree in a health-related field of counseling, psychology, social work, or other behavioral health field.
  • Candidates with a bachelor's degree in criminal justice must have fifteen college-level semester hours of coursework in behavioral health.
  • Valid Tennessee Driver's License.
  • Acceptable Motor Vehicle Report (MVR).
  • Acceptable Criminal Background Investigation.
  • Personal Automobile Insurance.
  • Cell Phone Transportation that seats 4 people.
