About the position

The Intensive Case Manager works under the direction of the Director of the Community Mental Health Center, providing intensive case management services to clients in various mental health programs. The role focuses on assisting clients in accessing mental health and substance abuse treatment, securing housing, and achieving independent living through comprehensive support and planning.

Responsibilities

  • Provide intensive case management services to clients, including assistance with mental health and substance abuse treatment, benefits applications, housing, education, and job placement.
  • Consult with CMHC and other mental health professionals to develop individual treatment plans.
  • Establish and maintain effective working relationships with clients, supervisors, County employees, attorneys, law enforcement, judges, and other agencies.
  • Comply with Civil Service policies, collective bargaining agreements, and County policies to ensure a safe and respectful work environment.
  • Recommend admission to the diversion program to the County Attorney's or City Prosecutor's Office.
  • Support participants in achieving independent living and assist them in planning for future needs and goals.
  • Plan, develop, and provide intensive social work services.
  • Make professional decisions and maintain confidentiality.
  • Escort and transport clients to appointments and meetings.
  • Prepare and maintain various records, reports, and documentation in accordance with CMHC standards and legal requirements.
  • Participate in the development of policies and procedures with other health professionals.
  • Safely operate a motor vehicle for County business as required.
  • Report to work with regular, consistent attendance.
  • Perform other duties as assigned.

Requirements

  • Bachelor's degree in Social Work, Criminal Justice, Psychology, Human Services, or Behavioral Health.
  • Two years of experience in case management within the mental health, human services, or social services field.
  • Basic life support/cardiopulmonary resuscitation certification required at the time of hire and throughout employment.
  • Valid driver's license required at the time of hire and throughout employment.
  • Completion of a pre-employment criminal background check and conditional offer drug screen required.

Nice-to-haves

  • Experience working with the criminal justice system or chemically dependent populations.
