About the position

The Intensive Community Services Support Specialist plays a crucial role in promoting community stabilization through case management activities tailored to the needs of consumers. This position involves in-home and community-based mobile case management, focusing on illness management, peer support, crisis prevention/intervention, and the effective use of community resources. The specialist is responsible for conducting assessments, developing individualized service plans, and providing direct support to individuals to enhance their independence and engagement in mental health services.

Responsibilities

  • Complete comprehensive strength-based psychosocial assessment within 7 days of admission.
  • Collect information and develop Individual Program/Service Plan (ISP) within 7 days of admission.
  • Maintain clinical supervision of ISP and ongoing assessments, including risk assessments and safety contracts.
  • Update ISP every 30 days.
  • Schedule and attend a minimum of six face-to-face contacts or six hours of direct contact per month.
  • Monitor medication compliance and assist in structuring self-medication regime.
  • Provide family/support system education and support.
  • Provide direct active rehabilitation and teaching through problem-solving and crisis intervention.
  • Develop and implement strategies to ensure individual engagement in mental health treatment services.
  • Assist in transitioning to lower level of care and increased community independence.
  • Perform services coordination and case management activities.
  • Participate with and report to treatment/rehabilitation team on individual's progress.
  • Respond rapidly to crisis situations and participate in crisis/relapse intervention.
  • Facilitate the individual's transition back into the community upon discharge if hospitalization is necessary.
  • Complete all required case file documentation.
  • Assist in maintaining case records, compiling statistics, and writing reports as requested.
  • Provide on-call coverage rotation and address situations as warranted.
  • Complete 15 hours of intensive ongoing training per year.

Requirements

  • High school diploma or equivalent required.
  • At least two years of lived experience, direct care experience in a human service field, training in a human service field, or a bachelor's degree in a human service field.
  • Ability to be flexible and work on multiple projects or tasks simultaneously.
  • Good communication skills, both written and spoken.
  • Demonstrated ability to handle confidential matters.
  • Knowledge of Microsoft Office Suite and web-based systems for payroll and data access.
  • Sensitivity to the service population's cultural and socioeconomic characteristics.
