Responsibilities
- Provide skilled nursing care to individuals with disabilities in an Intermediate Care Facility setting.
- Implement and oversee the health care plans for residents.
- Administer medications and perform medical procedures as needed.
- Collaborate with a multi-disciplinary team to ensure the well-being of residents.
- Maintain accurate medical records and documentation.
- Offer support and guidance to Direct Support Professionals (DSP).
- Participate in complex case management and engage in continuous learning opportunities.
Requirements
- Valid License Vocational Nurse (LVN) license in the state of California.
- CPR certification.
- Strong communication and interpersonal skills.
