About the position

The Physical Therapist at JAG Physical Therapy is responsible for assessing patients' strengths and weaknesses, creating effective treatment plans, and providing high-quality care in a supportive environment. This role emphasizes patient confidentiality, positive encouragement, and collaboration with an interdisciplinary team to maximize patient recovery and functional independence.

Responsibilities

  • Collect and record patient care data following guidelines and regulations.
  • Prepare for evaluations by obtaining patient history and understanding diagnoses.
  • Select and administer tests and measures to formulate diagnosis and treatment plans.
  • Establish treatment plans based on evaluations and best practices.
  • Collaborate with interdisciplinary teams and patient support systems.
  • Assess and modify treatment plans as necessary.
  • Provide education and training to patients and their support systems.
  • Oversee treatment and exercise prescriptions for patient caseloads.
  • Provide safe and effective interventions in compliance with JAG standards.
  • Document all treatments using Prompt EMR and ensure accurate billing.

Requirements

  • Bachelor's or Master's Degree required, Doctorate degree preferred.
  • Licensed Physical Therapist in the state of practice.
  • Completed state regulated CEU requirements.
  • Valid CPR License.
  • Experience with EMR software.
  • Excellent oral and written communication skills.
  • Strong clinical decision-making skills.
  • Excellent work ethic and dedication to patient success.
  • Basic computer skills.
  • Ability to work as a team member and independently with minimal supervision.

Nice-to-haves

  • Experience in outpatient orthopedic physical therapy.
  • Familiarity with community reintegration practices.
