About the position

The Jail Transition Caseworker plays a crucial role in facilitating the transition of individuals from the Nolan County Jail to community resources, ensuring continuity of care for those with mental health or substance abuse disorders. This position involves coordinating behavioral and physical health needs upon release, providing case management, and engaging with various stakeholders to support individuals in their reintegration into the community.

Responsibilities

  • Coordinate behavioral and physical health needs upon release from jail.
  • Provide behavioral health consultation and follow-up with primary care and behavioral health care patients.
  • Perform case management activities including service linkage and behavioral health assessments documentation.
  • Coordinate clinic services and monitor patient progress.
  • Advocate for patients and assist in crisis resolution.
  • Serve as a liaison between jail staff, community members, and healthcare providers.

Requirements

  • Bachelor's degree from an accredited college or university with a major in social, behavioral, or human services.
  • Valid Texas driver's license with an acceptable driving record for insurability.
