About the position

The Jail Transition Specialist at Catholic Charities Serving Central Washington plays a crucial role in providing case management and clinical services to individuals with mental health disorders within the criminal justice system. This position focuses on facilitating release planning and coordinating care through the Sequential Intercept Model, ensuring that clients receive the necessary support and resources during their transition from incarceration to community living.

Responsibilities

  • Provides quality Mental Health services to consumers with a mental health disorder who are in the criminal justice system within the Catholic Charities Services area.
  • Completes progress notes and other required documentation in the electronic consumer files within the required timeframe.
  • Provides services to or on behalf of the person served.
  • Services may be provided in a jail or juvenile detention setting and in the community or in one of the program locations.
  • Services may include Crisis Intervention, assisting with applying for benefits, activities of daily living, psychosocial support, service coordination, linkage, transportation, medication monitoring, behavioral health counseling, accessing community resources and/or housing/housing retention.
  • Establishes and maintains working relationships with community resources, coordinates consumer treatment with other providers, and initiates referrals to medical providers and other community resources when appropriate.
  • Performs other duties as assigned.

Requirements

  • Bachelor's degree in Behavioral Sciences or Social Work preferred.
  • Associate's degree in Behavioral Sciences or Social Work required.
  • 2 years experience in direct Clinical treatment services preferred.
  • 1 year experience working within the criminal justice system preferred.
  • Strong family and individual Clinical Case Management skills.
  • Possess and utilize effective verbal and written communication skills.
  • Ability to be self-motivated, work independently and as a team member.
  • Good understanding of Catholic Charities policies and procedures.

Nice-to-haves

  • Bilingual (English/Spanish) preferred but not required.
