About the position
The Medical Office Coordinator (MOC), also known as the Care Transitions Coordinator (CTC), plays a crucial role in providing administrative support to the Medicine service at Johns Hopkins Bayview Medical Center. This position involves interacting with patients, their families, referring physicians, and third-party carriers to address patient-related issues, ensuring smooth care coordination and maintaining positive patient relations.
Responsibilities
- Contact patient's primary care provider (PCP) office to inform them of patient's admission and covering provider.
- Schedule phone meetings for PCP and covering providers to update on patient status.
- Obtain patient medical records and call pharmacy to obtain patient medication lists for medication history and reconciliation processes.
- Assist with the coordination of collaborative patient rounding for doctors and nurses.
- Coordinate with case management and social work to notify nurses that the discharge process is complete.
- Support the physician with insurance preauthorization by submitting necessary clinical information.
- Confirm that pending inpatient discharge tasks are being completed.
- Remind providers to monitor the signing of daily notes and/or discharge summaries according to policy.
- Pick up patient medications from pharmacy when a Meds for Home Coordinator is unavailable.
- Facilitate the scheduling of family meetings with case management and providers.
- Assist with administrative paperwork associated with coordination of patient transfers to other sites.
Requirements
- High School Diploma or GED required.
- Additional education may substitute for required experience.
