About the position

The JP Court Case Manager position is responsible for providing assessment, monitoring of court-ordered conditions, and case management services to at-risk youth referred by various school districts and JP Courts. This role involves facilitating family team meetings, coordinating services, and maintaining regular contact with families to strengthen relationships and address their needs. The position operates under the supervision of the JP Wraparound Supervisor and the JP Court Program Manager, contributing to the welfare of children and families in Harris County.

Responsibilities

  • Provides assessment, monitoring of court-order conditions, and care coordination utilizing the Wraparound process to youth.
  • Facilitates family team meetings and coordinates services with youth and families.
  • Maintains regular face-to-face contact with the family, including meetings in homes, schools, and community.
  • Facilitates and assists in the development of individual plans of care for each youth and family that is strength-based.
  • Serves as a liaison to initiate and maintain communication between various agencies and service providers.
  • Processes and maintains a complete record of referrals, contacts, and meeting information in Evolv for all assigned cases.
  • Provides information for monthly reports and records to appropriate agencies.

Requirements

  • Bachelor's degree in Psychology, Sociology, Social Work, Criminal Justice, or closely related field.
  • One (1) year full-time paid continuous work experience working with youth and their families in a youth-service agency or social services setting.
  • Training and experience using a personal computer with software such as Microsoft Word, Microsoft Excel, and Case Management Software.
  • Reliable transportation with a valid driver's license (Texas) and auto liability insurance.

Nice-to-haves

  • Knowledge of protective services, child and family development, and family systems.
  • Knowledge of county, state, and federal laws, Texas Family Code, and regulations governing children.
  • Ability to maintain high ethical standards and handle sensitive situations calmly.
  • Skill in analyzing and applying Texas Family Code policies and procedures.
