About the position

The JP Court Case Manager position at Harris County involves providing assessment, monitoring, and case management services to at-risk youth referred by school districts and JP Courts. The role is supervised by the JP Wraparound Supervisor and the JP Court Program Manager, focusing on facilitating family team meetings, coordinating services, and maintaining communication with various stakeholders to support youth and their families effectively.

Responsibilities

  • Provides assessment, monitoring of court-order conditions, and care coordination utilizing the Wraparound process to youth.
  • Facilitates family team meetings and coordinates services with youth and families to maintain and strengthen their relationships and address underlying needs.
  • Maintains regular face-to-face contact with the family, including meetings in homes, schools, and the community.
  • Facilitates and assists in the development of individual plans of care for each youth and family that are strength-based.
  • Serves as a liaison to initiate and maintain communication between collaterals, including the Harris County Juvenile Probation Department and other service providers.
  • Processes and maintains a complete record of referrals, contacts, and meeting information in Evolv for all assigned cases.
  • Provides information for monthly reports and other written materials to appropriate agencies.

Requirements

  • Bachelor's degree in Psychology, Sociology, Social Work, Criminal Justice, or a closely related field.
  • One year of full-time paid continuous work experience working with youth and their families in a youth-service agency or social services setting.
  • Training and experience using Microsoft Word, Microsoft Excel, and case management software.
  • Reliable transportation with a valid Texas driver's license and auto liability insurance.
  • Knowledge of protective services, child and family development, and community resources.

Nice-to-haves

  • Experience in case management documentation and records management.
  • Familiarity with the Justice of the Peace system procedures and legal terminology.
