About the position

The Junior Coder is responsible for coding medical records accurately, including all diagnoses and operative and diagnostic procedures, using established coding systems such as ICD, CPT, and HCPCS. This role involves entering coded information into an automated grouper system and ensuring compliance with coding standards. The Junior Coder also engages with hospital staff to clarify medical documentation and contributes to the overall management of medical records.

Responsibilities

  • Assign and record accurate codes to all diagnoses, procedures, and operations as documented by the attending physician.
  • Ensure all factors necessary for assigning an accurate DRG are present and that all diagnoses are ranked properly.
  • Make appropriate contacts to acquire or clarify necessary information.
  • Enter final diagnostic code numbers and narrative descriptions into an automated grouper system.
  • Provide information and respond to inquiries regarding medical documentation and DRGs from hospital staff.
  • Abstract information from medical records to compile reports and statistical information.
  • Enter data such as diagnosis, treatment, admission and discharge dates, and length of stay into an automated database.
  • Participate in other medical records functions as assigned.

Requirements

  • CPC, CPC-A, COC, CCS or CCS-P credential preferred.
  • Two years of experience working with medical records in a health care setting, or satisfactory completion of at least 30 college credits including courses in medical terminology and ICD coding, or a college certificate program in medical records coding.
  • High school or equivalency diploma required.
  • Good knowledge of medical terminology and working knowledge of the principles of the medical record system.
  • Ability to understand and code medical records effectively.
  • Effective communication skills, both orally and in writing.
  • Proficiency in computer applications such as spreadsheets, word processing, and database software.

Nice-to-haves

  • Experience with automated grouper systems.
  • Knowledge of hospital coding compliance and regulations.
