About the position

The Kidney Care Advocate I position at Fresenius Medical Care is designed to support patients living with kidney disease by providing education on treatment options and facilitating their transition into kidney care services. This role involves collaboration with care teams to ensure a positive patient experience, assisting with home dialysis education, and maintaining communication with healthcare providers.

Responsibilities

  • Collaborates with Regional Home Therapy team to provide administrative support and ensure comprehensive education on modality options for patients and caregivers.
  • Under supervision, provides information regarding renal replacement therapy options to patients with end-stage renal disease (ESRD).
  • Assists patient care teams in navigating patients through the initiation of dialysis and transitions to home therapy.
  • Maintains communication with physicians and interdisciplinary team members to coordinate patient referrals and admissions.
  • Identifies and resolves barriers to expedite home therapy training and admissions.
  • Promotes Kidney Care Advocate services and educational programs to providers and the community.
  • Ensures accurate documentation of patient interactions and status in the SAP database.

Requirements

  • High school diploma or GED; college degree preferred.
  • Minimum of one year experience in patient interaction, teaching, or sales.
  • Demonstrated customer service skills and ability to work autonomously.
  • Excellent verbal, written, and presentation skills.
  • Proficient in Microsoft Outlook, Excel, Word, PowerPoint, and smart devices.

Nice-to-haves

  • CCHT or LPN certification preferred.
  • Experience in dialysis preferred.
  • Familiarity with Salesforce or Customer Relationship Management systems.
