About the position

Cascade AIDS Project (CAP) is the oldest and largest AIDS Service Organization (ASO) in Oregon and Southwest Washington that provides the LGBTQ+ community and beyond with compassionate healthcare. CAP's mission is to promote well-being and advance equity by providing inclusive health and wellness services for LGBTQ+ people, people affected by HIV, and all those seeking compassionate care. Prism Health, Our House, Tod's Corner and Esthers Pantry are all a part of CAP. Cascade AIDS Project is excited to announce that we are hiring skilled and team-oriented Latine/x Services Navigator. The MAI (Minority AIDS Initiative) Latine/x Services Navigator provides strengths-based social services to people living with HIV/AIDS (PLWHA) who are Latine/x and assists participants to navigate the medical care, mental healthcare, and substance use systems and provide support to clients in connecting to housing case management services, as this is critical to their stability and medical engagement. This is a Full-Time role working at our Davis St location in the Old Town area of Portland but will travel throughout the five-county service area for meetings, client home-visits and healthcare appointment. The schedule for this role is Hybrid Schedule, 9am-5pm with some nights and weekends required.

Responsibilities

  • Provide HIV-related services navigation for Latine/x clients
  • Make appropriate referrals to CAP programs, mental health, substance abuse, HIV prevention, or other community partners as needed to support client self-sufficiency
  • Carry an active client caseload of approximately 30 MAI clients and work in collaboration with housing case managers, medical case managers, medical providers, or other providers involved in client care to provide coordinated comprehensive care to PLWHA with a focus on medical engagement, medication adherence, health literacy and housing stability
  • Compile and coordinate information on local alcohol and drug, mental health, food, clothing, and other resources for clients and share the information with colleagues
  • Attend confirmatory meetings for newly diagnosed Spanish-speaking clients to discuss linkage to care options and follow up navigation services
  • Develop curriculum and facilitate a monthly culturally specific support group. This includes tracking a list of attendees and fielding referrals for the group.

Requirements

  • Verbal and written fluency in English and Spanish
  • A minimum of one year experience providing case management (or similar) services
  • Demonstrated ability to effectively collaborate with community stakeholders
  • Good organizational and time management skills
  • Able to travel throughout the service area on a frequent basis
  • Available to work evenings and weekends
  • Two years' experience relevant to the position or bachelor's degree in human/social services field (social work, public or community health, psychology) or related field
