About The Position

The Latino Chronic Disease Cohort Manager at Harris County Public Health will oversee a team of coordinated care professionals to enhance public health outcomes for the Latino community. This role involves strategic leadership, program oversight, community engagement, and ensuring compliance with health initiatives. The manager will lead outreach efforts, coordinate care services, and foster collaboration among various stakeholders to improve access to health resources for residents of Harris County.

Requirements

  • Bachelor's degree in Business Administration, Healthcare Administration, Public Health, or a related field.
  • Two (2) years of supervisory experience leading teams in the social service space.
  • One (1) year of experience working as a Case Manager or in Case Management.
  • One (1) year of experience working with coordinated care.
  • Valid Driver's License (TX upon hire).
  • Proficient in Microsoft Office Suite software, including 365 programs.

Nice To Haves

  • One (1) year of experience with chronic diseases within the Latino population.

Responsibilities

  • Provide leadership and guidance to the Cohort Managers, ensuring alignment with organizational goals.
  • Oversee the operations of the care coordination team, ensuring effective service delivery for participants enrolled in ACCESS.
  • Act as a liaison between the ACCESS Harris IMDT Manager, care coordination team, and relevant departments.
  • Supervise and guide the care coordination team, ensuring efficient and trauma-responsive case management services.
  • Regularly evaluate and document team performance, providing continuous feedback and facilitating improvement.
  • Develop and implement outreach strategies to raise awareness of ACCESS Harris services.
  • Conduct community assessments to identify resources and organizations that can support ACCESS Harris goals.
  • Ensure adherence to internal policies and external regulations.
